Complete the new client form. Once you have completed the form an invite to my client portal will be sent to you by email. This will help us determine whether I am the right fit for you. If you are using insurance please have the following information available, member ID, group policy, co-payment, deductible, and limit of sessions.

Login into your client portal to complete all intake paperwork, sign, and upload documents. You will be required to upload the following information: A photocopy of your ID and a copy of the front and back of your health insurance card, if you are utilizing your health insurance. Medical records, referrals from your PCP, Counselor or employer.
