Step One: Request an appointment by going to the client portal. When selecting your appointment make sure you give yourself enough time to complete all the onboarding paperwork. Once you have requested an appointment, and your appointment has been approved. You will receive a invite from Therapy Portal.

Step Two: Please login the client portal to complete all forms and upload all required documents on the client portal. Once you have been invited to the therapy portal. You will have 48 hours to complete all onboarding forms. Your appointment will be cancelled if your onboarding paperwork is not completed. Please check your spam box in case you have not received a response. If you have not received a responds in 48 hours please contact the office at 210-895-4315 by phone or text.

Step Three: Call your health insurance to verify your benefits. The phone number should be listed on the back of your insurance card. You will need to know the following information.

Do you have a deductible that needs to be met? if so what is the amount.

Do you have a co-payment? If so what is the amount you are expected to pay per session.

Are there limits to the amount of sessions you can have per year?

Do you need a prior authorization to see a counselor? If so from whom.

It is important for you to understand and know what your health insurance benefits coverages are. It is your responsibility to pay for any services your insurance will not cover.

Step Four: (Optional) For a collaborative approach, please make sure you complete the release of information form with your primary care physician, psychiatrist, or any other professional who is managing your mental healthcare and/or medications.

Please have your doctor who is treating your mental health fax records and medication list to Fax number: 210-899-1415.

Please complete the Telehealth consent form. This form is required. This is required when you are seeing your counselor in different location other than your home. Notice of Teletherapy Practice Consent Form (will need to be completed every time you change the location of your telehealth session; if the address is not is listed on the form).
