Complete the Health Insurance Verification & Screening Form Form. Once you have completed the forms an invite to my client portal will be sent to you by email. If you are using insurance please have the following information available, member ID, group policy, co-payment, deductible, and limit of sessions.

Set up your client portal. Please follow the instructions in your therapy portal invitation email on how to sign up for a client portal with counselor Jennifer L. Hillier M.A. LPC. Please check your spam box in case you have not received a response. Login into your client portal to complete all required forms. Sign, and upload documents. You will be required to upload the following information: A photocopy of your ID and a copy of the front and back of your health insurance card, if you are utilizing your health insurance. medical records, referrals from your PCP, counselor or EAP authorization from your employer. Please make sure you complete the release of information form, if your primary care physician or psychiatrist is managing your mental health medication. Please have your doctor who is treating your mental health fax records and medication list to 210-899-1415.

Once you have set up your client portal you will be allowed to select an appointment. If the appointment is available an confirmation of your appointment email will be sent to you or an request another date and time will be sent if the appointment is not available.
