Step Three: Call your health insurance to verify your benefits. The phone number should be listed on the back of your insurance card. You will need to know the following information.
Do you have a deductible that needs to be met? if so what is the amount.
Do you have a co-payment? If so what is the amount you are expected to pay per session.
Are there limits to the amount of sessions you can have per year?
Do you need a prior authorization to see a counselor? If so from whom.
It is important for you to understand and know what your health insurance benefits coverages are. It is your responsibility to pay for any services your insurance will not cover.
Please complete the Telehealth consent form. This form is required. This is required when you are seeing your counselor in different location other than your home. Notice of Teletherapy Practice Consent Form (will need to be completed every time you change the location of your telehealth session; if the address is not is listed on the form).
